New Patient Information
Welcome to Flowers Medical Group and thank you for choosing us to serve your health care needs. The more you know about our policies and methods of practice, the better we may be of service to you. Should you have any questions, please do not hesitate to contact us. It is our goal to provide you and your family the best medical care possible and to maintain a pleasant and long-lasting relationship with each of you.
In preparation for your office visit, new patients should arrive 30 minutes early to complete patient forms, etc. Other patients need to arrive 15 minutes early or as directed by staff at appointment time.
Please bring the following items with you to your appointment:
- Valid driver license or identification card
- Your insurance card(s)
- Referral form(s), if required by your insurance company
- Pertinent medical information and imaging studies needed for office visit
- A list of all medications currently taking including strength and dosage
- List of all your concerns so that you may discuss them with the physician
- Completed patient forms
Be prepared to pay your co-payment, if applicable.
